Financial Expectations

Thank you for choosing us as your behavioral health care provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered a part of your treatment. The following will help you understand Affiliated Clinical Services Financial Policy.

All patients must complete our information and insurance form before seeing the doctor/therapist though our intake department.

Insurance Benefits:

Our Intake department staff will work with you to help you get answers concerning insurance coverage for treatment at Affiliated Clinical Services. With your permission, an intake specialist will contact your insurance carrier to request a quote of your insurance benefits as part of the screening process. Once we receive the quote of benefits, our intake staff will call you to explain this quote and explain what you will owe for your session(s) based on the quote. Please keep in mind this quote is not a guarantee of coverage; that is why we strongly encourage you to contact your insurance carrier to discuss your plan’s coverage and to refer to your benefit plan handbook regarding what is covered.

Payment:

All copayments and deductibles are due at the time of service. These payments will be based off of the quote of benefits, which our intake department received from your insurance company.

We accept cash, checks, money orders, and credit cards; Visa, MasterCard and Discover.

We do not offer payment plans for any AODA services because of the frequent visits. We feel that payment of your services is a part of your treatment here and due to the fact that you will be seen so frequently in this program large balances accumulate quickly and we do not want that to become a detriment or cause disruption to your treatment.

Regarding Insurance:

We will submit all claims to your insurance company, however the balance is your responsibility whether or not your insurance company pays. Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. Please be aware that some and perhaps all of the services may be non-covered services and not considered reasonable and customary under your medical insurance policy.

Affiliated Clinical Services requires that you make us aware of all changes to your insurance on or before the changes take place. If you do not bring your insurance card along with you at the time of service, or do not make us aware of any changes to your insurance and your insurance denies your claim, you will be responsible for paying the charges. If the change took affect more than 3 months ago, you will be responsible for paying the services and you will need to bill your insurance company. If you need to get authorization prior to seeking services, and have not done that and your insurance denies your claim, you will be responsible for that charge.
