Financial Expectations
Thank you for choosing us as your behavioral health care provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered a part of your treatment.
All patients must complete our information and insurance form before seeing the doctor/therapist though our intake department. We help to make it as easy as possible. A patient account representative will assist you in verifying your insurance coverage and help determine any required deposits toward your deductible or co-payments. We also highly recommend that you contact your insurance carrier personally so you can fully understand your benefits. We do ask that clients make co-payments and deductible payments at the time of service.
Affiliated Clinical Services accepts most commercial insurances. We accept Medicare, however, we are unable to accept Medicaid.
Rates vary based upon the service provided, the length of the appointment and the programs for which you qualify. Our Intake Representative will review this information with you when you call for your appointment, or you may contact one of our Customer Service Representatives at (262) 338-2717 for specific questions.
